Housing benefit and council tax reduction forms

 

The list below includes application forms and supporting forms for housing benefit and council tax reduction. You can download and print the forms, but they all require a signature before being returned to us.

Alternatively, you can call us on 0344 980 3333 or email benefits@norwich.gov.uk and we will send you the forms you need.

Please provide all the supporting evidence as we cannot pay your benefit until we have seen all the evidence we have asked for.

Use this form to apply for housing benefit and council tax reduction.

Application for housing benefit before you move into your new home (92.4 Kb pdf)
Use this form if you have to pay rent before you move in and you cannot move in at the start of your tenancy. You will also need to complete an application for housing benefit and council tax reduction, if you haven't done so already. 

Application for housing benefit on two homes (90.1 Kb pdf)
Use this form if you have had to move to a new address and you still have to pay rent at your last address.

  Backdated housing benefit and council tax reduction (338 Kb pdf)
Use this form to make a claim for backdated housing benefit and/or council tax reduction. You will also need to complete an application for housing benefit and council tax benefit, if you haven't done so already. 

Certificate of earnings (95.5 Kb pdf)
Fill in this form with your employer to provide proof of your earnings if you are claiming housing or council tax reduction and do not have payslips yet, or if you have lost or destroyed the payslips we need to see. You will also need to complete an application for housing benefit and council tax reduction, if you haven't done so already.

Complete this form if you are moving to a new address and are claiming housing and council tax reductionand you currently live in Norwich.

Complete this form if you would like us to end your housing and council tax reduction claim. 

Nil income declaration (346.pdf)
Complete this form if you need to tell us that you are not receiving any income. You will also need to complete an application for housing benefit and council tax reduction, if you haven't done so already.

Use this form if you are a landlord and think that if your tenant receives direct benefit payments it will cause them, or you, serious problems.

Rent certificate (58.2 Kb pdf)
Use this form when there is no tenancy agreement or if the tenancy agreement is more that 52 weeks old. This form must be completed by your landlord. You will also need to complete an application for housing benefit and council tax reduction. If you have done so already then you need to complete a change of circumstances – change of address form.

Use this form if you are self-employed and are making a claim for housing benefit or council tax reduction. You will also need to complete an application for housing benefit and council tax reduction. If you already have a housing benefit or council tax reduction claim and your income from self- employment has changed, please use this form to tell us about it.

Use this form if we are recovering an overpayment from you and you would like to provide us with details of your income and expenditure so that we can work whether we need to reduce the overpayment recovery rate.
