Online forms: Request for Lord Mayor or Sheriff presence at events

Complete the form if you would like your event to be attended by a civic head.

Please make sure that you submit your request no later than three weeks prior to your event. Your request will be considered within 10 working days.

**You will also be asked to submit a risk assessment. This should relate to your event and the Covid-19 precautions you have in place to ensure the event goes ahead safely. We are aware this may be subject to change in line with current government requirements. We will not be able to confirm civic attendance at your event without this risk assessment**

Once you have started, you must continue to the end and submit the form. Please note that you only have 10 minutes to fill out each page and you will not be able to come back to the form at a later time because your answers will not be saved.
