Use this form to apply for a new Go4Less discount card, or to renew an existing Go4Less discount card.
You will need to provide:
- your details, including contact details and date of birth
- if you are renewing, your Go4Less card number
- details of any child dependants
- proof of address and eligibility
- photograph of each applicant, inluding any dependants
Before you start the form, please have a scan or photo of your documents of proof ready to upload and saved to your device. Files can be PDF, JPEG or Word documents, maximum size 4MB.
Once you have started, you must continue to the end and submit the form. Please note that you only have 30 minutes to fill out each page and you will not be able to come back to the form at a later time because your answers will not be saved.
