Report suspected food poisoning

Food poisoning is an illness caused by eating contaminated food. It's not usually serious and most people get better within a few days without treatment.

The symptoms of food poisoning usually begin within one to two days after eating contaminated food, although they may start at any point between a few hours and several weeks later.

Before you start

Please be aware that we are unable to investigate all reports of suspected food poisoning. We will review and record the information you have provided as part of our monitoring of food business premises. If your report is selected for further investigation we will contact you.

You will need to provide:

  • name and address of the food business
  • details of when and what you ate and your symptoms
  • if you contacted the business, name of contact and what action have they taken
  • your email address incase we need to contact you

Please note that when you start the form you must continue to the end and submit it. You only have 30 minutes to fill out each page and you will not be able to save the form and come back later.
