Apply for Housing Benefit before you move into your property
If you delayed moving into your accommodation this form can be used to apply for up to a maximum of four weeks benefit prior to moving in.

If you are unable to apply for Housing Benefit/Council Tax Reduction online use this form. This form must be signed.

Information to support your Housing Benefit/Council Tax Reduction award

Nil income declaration form
Complete this form if you:

  • get Housing Benefit or Council Tax Reduction
  • have no income or other benefits.

Include bank statements for all your accounts.

Secondary property form
If you own or part-own any property or land (in the UK or abroad) that is not the one you're claiming benefit for, please fill in this form with the details. We might contact you later if we need more information. 

Download self-employed earnings form
Use this form to report your self-employed income and expenses for the period specified. Include all relevant details and figures.

Tenant accommodation questionnaire
If you live in accommodation where you get care, support, or supervision and you're applying for Housing Benefit, you must also fill out this form. 

The form is online and submits automatically. If you can't complete it online, please contact us to request a paper version. 

Payment of Housing Benefit

Payment to landlord request form
Use this form if a tenant receives Housing Benefit and you believe it's best for the payments to go directly to the landlord. Also use it if the tenant has rent arrears of eight weeks or more. 

Landlords request to have Housing Benefits payments by BACS
If you are a landlord use this form where you request Housing Benefit payments. You will need to prove bank details. 

Tenant payment to landlord request form
Get your Housing Benefit paid to your landlord.

If you get Housing Benefit and want it paid straight to your landlord, fill in this form.

You’ll need:

  • Your landlord’s permission
  • Their bank details

Note: If you rent from Norwich City Council, your Housing Benefit will go straight to your rent account.

Tenant request to have Housing Benefit payment by BACS
Use this form if you want to change the bank account your Housing Benefit goes into.

  • You must be named on the account.
  • Send proof of the account, like a recent bank statement.

Norwich City Council tenants: Your Housing Benefit must go straight to your rent account. It can’t be paid into a bank account.

Non-residential carer benefit information and application form
If your Housing Benefit is reduced due to living in a larger property, we may be able to re-consider our decision where an additional room is required for a carer who stays overnight.

Statement of means form
Use this form if you have been overpaid Housing Benefit and would like to request a different payment arrangement. We will write to you with a decision after receiving the form.

Third party consent form
If you receive (or are applying for) Housing Benefit or Council Tax Reduction and want someone else to help you, you must complete a consent form before we can share any information.

You can withdraw your consent at any time by contacting us again.
