If you need to update the name or address of the premises licence holder or designated premises supervisor (DPS), you can use the notification form below.
Use this form only when the premises licence is still held by the same person or company, but their details have changed - for example, a change of name due to marriage.
If a new person or company is taking over responsibility for the licence, you will need to submit an application to transfer the premises licence, not this notification form.

The fee for amending a premises licence is £10.50.
You must return the full existing premises licence, including the summary, along with your completed notification form.
If you cannot return the licence, please provide a brief explanation of the reason.
