When to use this form
Use this form only when the premises licence is still held by the same person or company, but their details have changed - for example, a change of name due to marriage.
When you should not use this form
If a new person or company is taking over responsibility for the licence, you will need to submit an application to transfer the premises licence, not this notification form.

Fees
The fee for amending a premises licence is £10.50.
What you need to provide
You must return the full existing premises licence, including the summary, along with your completed notification form.
If you cannot return the licence, please provide a brief explanation of the reason.
