When you complete the form

You will need to provide:

  • your current licence number and expiry date (for renewal and variation)
  • the plan of your premises layout saved to your device as a scan or photo which you can upload (as a JPG, PDF or Word document, maximum size 4MB)
  • vet contact details
  • liability insurance policies
  • waste disposal arrangements
  • security details
  • disease control
  • fire safety precautions
  • details of any licence disqualifications and convictions
  • payment of the first part of the fee
