Use this form to book a support visit for your food business. We can provide general or specific food safety advice and help you improve or maintain your food hygiene rating.

Information you will need before you begin the form

A support visit costs £160. You will need to pay by credit or debit card when you submit the form.

Once we receive your booking, we will contact you to arrange the visit.

Before you start

Before you begin, please note:

  • You have 30 minutes to complete each page.
  • Your answers won’t be saved until you submit the form.
  • Once you start, you’ll need to complete the form in one go.
  • You won’t be able to leave and return later.
