- Poor hand hygiene
- Unsafe food handling practices
This form is for reporting hygiene and safety issues at food businesses only.
Issues relating to allergen labelling should not be reported using this form (see below for details).
Before you begin
We review all reports as part of our monitoring of food businesses. In some cases, we may contact you if further investigation is required.
Information you will need
Before you start, make sure you have the following details ready:
- The name and location of the business
- When you visited and a clear description of the issue you observed
- Details of any contact with the business, including:
- The date of contact
- The name of the person you spoke to
- What action they have taken
- Your email address (so we can contact you if needed)
You will also have the option to upload supporting evidence. We recommend having this ready before starting the form:
- Accepted file types: PDF, Word documents, or JPG images
- Maximum file size: 4MB
Allergen labelling
Problems with unavailable or incorrect allergen labelling should be reported to Trading Standards.
