Report a problem with a food product

Use this form to report a problem with food you have bought in the Norwich City Council area if you believe it could pose a food safety or health risk.

Examples include:

  • A foreign object found in your food
  • Food that appears to be contaminated (for example, with chemicals)
  • Food showing signs of mould, spoilage, or decay

This form is for reporting safety concerns about food products.

Before you begin

We review all reports as part of our work to protect public health. We may contact you if further information is needed or if your report requires investigation.

Information you will need

Before starting the form, make sure you have as much of the following information as possible:

  • The name and address of the business where you bought the food
  • Product details, such as the name, batch number, or expiry date (from the packaging, if available)
  • Full details of your concern
  • When and where you purchased the food, or the contact details of the person who purchased it
  • Your contact details (used only to contact you about your report and send a confirmation)

You will also have the option to upload supporting documents. We recommend having these ready before you begin:

  • Examples: receipts or photographs of the issue
  • Accepted file types: PDF, Word documents, or JPG images
  • Maximum file size: 4MB

Important

  • You must complete the form in one session
  • You have 30 minutes to complete each page
  • Your answers will not be saved until you submit the form
  • You cannot save your progress and return later
