Use this form to report suspected food poisoning linked to a food business.

Food poisoning is an illness caused by eating contaminated food. It is usually not serious, and most people recover within a few days without treatment.

Symptoms typically begin within 1 to 2 days of eating contaminated food, but they can start anywhere from a few hours to several weeks later.

This form is for reporting concerns about food businesses only.

Before you begin

We are unable to investigate all reports of suspected food poisoning.

We will review and record the information you provide as part of our monitoring of food business premises. If your report is selected for further investigation, we will contact you.

Information you will need

Before starting the form, make sure you have:

  • The name and address of the food business
  • Details of what you ate and when you ate it
  • Information about your symptoms and when they started
  • Details of any contact with the business, including:
    • The name of the person you spoke to
    • What action they have taken
  • Your email address (so we can contact you if needed)

Important

  • You must complete the form in one session
  • You have 30 minutes to complete each page
  • Your answers will not be saved until you submit the form
  • You cannot save your progress and return later
